The island of Alicudi is the furthest west of the Archipelago. Filicudi lies 10 nautical miles to the east, Stromboli lies 42 nm to the northeast, and Lipari lies 25 nm to the southeast. The island lies just north of the Sicilian towns of Cefalù and Sant'Agata di Militello.
Alicudi is the least developed for tourism of all the Aeolian Islands. The island has only one road of 200 meters that goes from the small pier to the helicopter landing pad. Mules are the primary source of transportation for goods from the shore to the dwellings on the hillside, which cover the eastern slopes.
